Once you’ve received your Perio360 Profile DNA oral microbiome submission kit, complete the sampling process as directed, and then enter the necessary information on the online form. You will need the patient information as well as the control number to complete the form. It is not necessary to include the form with the submission kit.

The submission kit (paper points placed in the paper envelope which is placed in the resealable poly bag) can be placed in any standard envelope and sent USPS first class mail. An address label is provided for your convenience.
